Details needed
- Identity copy of any one of the passport/ration cards / self-certified declaration (click for
format) with a photocopy of voter card / PAN card/driving license / nationalized bank issued by the
election commission related to identity. - Birth Certificate of Medical University.
- In cases where there is no hospital or hospital, even if there is no birth/death of the child in
the hospital, in such a case, in respect of birth/death certificate to the applicant, the village
head / regional councilor/ ma. MP / M.B., B. Signature of any one of the S doctor will have to be
submitted along with the certificate of Aweem.
